Drivers Licenses
Drivers License services are provided by the Suwannee County Tax Collector's
Office. An additional $5.25 service fee is charged for services provided
by The Tax Collector's Office. The following is a list of most of
the Drivers License Services we provide, with a brief description of
requirements. For answers to more specific questions please call us

Requirements for a Florida Drivers
License, or Florida ID card?

Original Operator Drivers License - First
Time License - Age 15 or Older 
- 2 forms of identification, a primary and secondary, as described
in information that follows. The names MUST match exactly.
- Traffic Law and Substance Abuse Education Course (TLSAE). This
course is provided by outside companies, and can be located by
looking in the phone book yellow pages, under Driver Instruction.
Driver Education Students may have a form, that will meet this
requirement, from their school.
- Under age 18 will need a parent with them to sign a Parental
Consent Form. This can only be done by a Natural Parent. If there
is a Guardianship, bring a Certified Copy of Court Documents,
showing there is Court Appointed Guardian.
- A written test is required, which includes 20 questions of road
rules, and 20 questions of road signs. The test may be waived
for Driver Education Students, with proper documentation from
their school.
- Social Security number is required
Non-Resident Drivers License - Original
Valid in Florida Only Drivers License 
- 2 forms of identification, out of state drivers license and one
other document, such as a social security card. The names MUST
match exactly.
- Vision test, with or without corrective lenses, to meet acuity
of 20/40.
- Hearing Test.
- Social Security number is required
NOTE: This non resident drivers license is for use only in the
State of Florida. When you travel outside of Florida, you will be
operating on your Out of State License. You will not be considered
a resident of Florida, and may not be eligible for some rights and
privileges of Florida Residents.
Original Commercial Drivers License (CDL)
- Transfer From Out of State 
- Two forms of identification, a primary and secondary, as described
on the list that follows. The names MUST match exactly. Usually
this is your out of state CDL and Social Security Card. Commercial
Drivers License Operators are required to present a Social Security
Card for issuance of a Florida CDL.
- Vision test, with or without corrective lenses to meet acuity
of 20/40.
- Hearing test.
- All tests and endorsements are reciprocated EXCEPT Hazardous materials.
Original Identification Card - All
First Time Issuances of a Florida ID Card. 
- Two forms of identification, a primary and secondary, as described
on the list that follows. The names MUST match exactly. ID Cards
can be issued as "Resident-Yes" or "Resident-No". A "Resident-Yes"
ID card will only be issued if you meet, and can prove one of
the following requirements:
- Surrender your out of state license.
- Children enrolled in the Florida School System.
- File a Declaration of Domicile with the Florida Court System.
- Proof of Employment in Florida. (A letter must be on employers
letterhead.)
- Social Security number is required
Renewal Information
Renewal of Florida Drivers License,
Commercial Drivers License (CDL) or Florida ID Card 
- Your expired Drivers License or ID card.
- Vision Test
Renewal of Valid in Florida Only Drivers
License 
- Your expired Florida License and your current Out of State License.
- Vision Test.
Lost Florida Drivers License or Lost Florida
ID Card 
- One form of identification. Something with your name on it,
as long as it is NOT a handwritten note.
Note: If your Drivers License or ID Card is expired for more than
one year, it may not be possible for us to find it on our system.
If your lost, Drivers License or ID Card is expired, you may be required
to comply with requirements pertaining to someone that has never had
a license.
Identification documents required for the
Florida drivers license or ID card 
PRIMARY IDENTIFICATION DOCUMENTS
Customers applying for an Original Florida Drivers License, or
Identification Card (ID Card) MUST present one of the following
documents:
- United States Birth Certificate (Original or Certified Copy),
from County or State Office in charge of holding such records.
Birth Certificates from hospitals or other medical institutions
are not acceptable.
- Valid United States Passport (unexpired)
- Alien Registration Receipt Card (Green Card) (Form I551)(unexpired).
- Employment Authorization Card, issued by the United States Department
of Justice, (Form I688b)(unexpired).
- Proof of Non-Immigrant Classification, provided by the U.S.
Department of Justice (Form I94)(unexpired).
SECONDARY IDENTIFICATION DOCUMENTS
The secondary document may be the original or a certified copy
of the following:
- School record stating date of birth, which must contain the
registrar's signature.
- Transcript of the birth record filed with a public officer,
charged with the duty of recording certificates.
- Baptism Certificate, which shows date of birth and place of
baptism.
- Family Bible Record or Birth Announcement in a baby book.
- Life Insurance Police on the customers life, which has been
in force for at least 2 years, and has a month, day, and year
of birth.
- Military or military dependent identification card.
- Florida or Out of State or out of country drivers license, or
ID (Valid or Expired).
- Florida license record or Identification record.
- Selective service registration (Draft Card).
- Florida Vehicle Registration Certificate, or out of state registration
certificate, if name and date of birth are shown.
- Receipt copy of your last Florida Drivers License issuance.
- Immigration Form I571.
- Federal Ford DD214 (Military Record).
- Marriage Certificate.
- Court Order which includes legal name.
- Florida Voter Registration Card, which has been issued in the
past 3 years.
- Social Security Card or certified receipt from SSA stating that
your social security number is verified as ___ ___ ___.
- Parent consent form of minor singed by the responsible adult.
NOTE: Only Certified Copies of Documents are accepted, and
ALL documents presented MUST be of the same name. Names that do
not match MUST have Marriage Certificate, Divorce Decree, or Legal
Name Change Ordered by the Court, signed by a Judge, which shows
the name from birth to current.
To apply for a hardship license
you must contact your local office. You must use the application
for hardship license form.
